Position Summary: This is a management position that oversees interdisciplinary teams supporting our C3 programs across two Carewest sites, and our Day Hospital program at a third community site. Both C3 and Day Hospital are community-based programs. The role will include overseeing day to day operations, innovative program planning, implementation and evaluation that meet the needs of our clients and families, effective and effective operational utilization of resources, and strong collaboration with internal and external stakeholders.

Program Summary: The Comprehensive Community Care (C3) program is a unique, long-term community-based program for seniors who require complex support while living at home. This is a highly integrated and coordinated program and offers access to a 24/7 healthcare team through a medical health clinic, day program, home support, access to respite beds, some limited transportation, and emergency response telephone system. For the C3 program, there are two hubs of support located at Beddington Towne Centre and our Sarcee care facility.

The Day Hospital program provides assessment and treatment for older adults who live in the community but require short-term, focused, goal-oriented care for issues that require care by an interdisciplinary team. Physical rehabilitation is a key component, along with supports for medical, mental, and/or social problems that prevent the client from living well at home. The program is located at the Glenmore Park facility.

Qualifications:

  • Bachelor’s degree in a health-related discipline required; Master's degree preferred.
  • Minimum of 5 years in a clinical operational healthcare leadership role.
  • Current registration with appropriate professional licensing college.
  • Reliable vehicle and driver’s license required.

  • Demonstrated ability to manage change and create innovative solutions for complex and diverse issues.
  • Demonstrated ability to communicate well with both written and verbal communication, and to engage effectively with internal and external stakeholders.
  • Leadership skills, including demonstrated ability and comfort with decision making responsibilities, coaching and teaching, and the ability to inspire and build confidence in others.
  • Commitment in building teams towards a common purpose. Proven ability to foster partnerships and to achieve organizational goals within a large, diverse organization.
  • Ability to foster a climate of cooperation amongst, and build relationships with, foundations, committees and other partners.
  • Knowledge and experience in the application of conflict resolution strategies.
  • Client and Family focused with a commitment to providing high quality services.
  • Ability to continually analyze effectiveness of strategies and implement quality and process improvements.
  • Demonstrates respect for culture and diversity.
  • Able to demonstrate competence in the development of new programs and services to meet the needs of the population.
  • Previous Home Care, Continuing Care, Transition Services or Ambulatory care experience.
  • PROSCI change management modules or certification.
  • LEAN, Six Sigma, or AHS AIW modules.
  • Accreditation experience.
  • Capital planning project experience.
