Cedar Woods Care Management is one of Philadelphia's most beloved supports coordination agencies, providing home and community-based services for adults with physical disabilities. We have provided FREE assistance to more than 2,000 adults in need, and our numbers are growing daily. Our specialty is providing support coordination for home and community-based services for adults with physical disabilities, but our consumer's whole well-being is our goal.

At Cedar Woods Care Management, our support coordinators work one-on-one with our consumers throughout the FREE application process for the Medicaid Home and Community Based Waiver Program, funded by the Commonwealth of Pennsylvania. All eligible applicants in Philadelphia and its surrounding counties receive an individualized service plan and continue to secure life-long support services.
