Sarah Axelrath serves as a Primary Care and Addiction Medicine Physician at the Colorado Coalition for the Homeless, focusing on full-spectrum adult primary care, low-barrier substance use disorder treatment, and street medicine outreach. Prior to this role, Sarah completed an Addiction Medicine Fellowship at Massachusetts General Hospital, where involvement included inpatient consultation and outpatient follow-up care, as well as collaboration with Boston Healthcare for the Homeless. Sarah's experience also encompasses an Internal Medicine residency and significant contributions to harm reduction initiatives through AmeriCorps VISTA and case management for homeless populations. Sarah holds a Doctor of Medicine degree from the University of Colorado School of Medicine and a Bachelor of Arts degree from Oberlin College, with additional studies at Yale University.

The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for families and individuals experiencing or at-risk of homelessness throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves. Since its founding more than 30 years ago, the organization has earned state and national recognition for its integrated healthcare, housing, and service programs. The Coalition’s comprehensive approach addresses the causes of homelessness, as well as the consequences, offering critical assistance to over 18,000 individuals and families each year. Positions at the Coalition span more than 50 programs and offer a variety of employment opportunities in a trauma-informed environment.
