Community Connections
Chad Holm, LPC is the Director of Assertive Community Treatment (ACT) at Community Connections and leads the agency’s On-Call Team for after-hour clinical services. ACT is the agency’s largest clinical program, with seven teams and approximately 75 employees, and provides the most intensive level of community-based mental health services to over 700 adult and young adult consumers. Chad received his BA and MA in Psychology/Counseling Program from California State University Sacramento and is a Licensed Professional Counselor in Washington, DC.
