Audrey Hamilton is a seasoned professional in the field of behavioral health care coordination, currently serving as a Behavioral Health Care Coordinator at SFA Community Health Network since September 2018. In addition to this role, Audrey has been the Behavior Healthcare Administrative Coordinator at Stephen F. Austin Community Health Network since August 2015, where responsibilities include providing case management for mental health patients and collaborating with counselors and psychiatrists. Prior experience includes serving as a Senior Office Associate at Texas A&M University at Galveston, focusing on Counseling and Career Services from March 2011 to July 2015. Audrey holds an Associate's Degree in Accounting from San Jacinto College.

MyCHN is a Federally Qualified Health Center (FQHC) in Brazoria, Galveston, and Harris County, Texas, that provides professional growth and development in a group practice providing primary health care for adults and children, women’s health services, behavioral health, and general dentistry. MyCHN is dedicated to continuous quality improvement and the ideal candidate will work within a team to ins e comprehensive and excellent care. MyCHN clinics are dedicated to Patient Centered Medical Home (PCMH) principles and practices. In 2015, MyCHN elected to develop an open source electronic health record and modify it to support the needs of an FQHC. MyCHN partnered with Williams Medical Technology in this endeavor which required significant additions to the system; including, design, documentation, reporting, workflow, and billing. MyCHN's key staff members and Williams Medical Technology programming staff meet at least weekly to further the development of interoperability, clinic workflow, decision support systems, data warehouse, and patient communications.
