Home Hospice is a community-based home health and nursing facility. Medical directors and nurse practitioners who make house calls, hospice-trained nurses, nurse aides, social workers, chaplains, bereavement counselors, volunteers, and others are all part of their circle of care. They provide medical care and assistance to patients, and also theirfamilies and loved ones. They also cover illness-related drugs and comfort-care meds. Patients in private homes, nursing homes, and inpatient hospitals received them. Respite care can also be arranged to help caregivers and families of terminally ill patients.
