Established in 2017, Impact Health PC is a physician owned, independent nurse practitioner group. Our consultative model provides care to patients within skilled nursing facilities and beyond through our telemedicine division. Staff consult with patients onsite for medical and psychiatric care during a residential facility admission and continue patient care after discharge to home with tele-medicine and remote patient monitoring services. Impact Health PC’s combined pre and post discharge focus avoids unnecessary hospital admissions to improve key quality metrics while enhancing patient and family satisfaction.
