Lifesong Hospice completes the circle of care the Leader family has been providing.  Lifesong is about living each day with grace.

Each of our residents lead a unique life. Lifesong will personalize hospice care plans to help our resident’s transition to be as comfortable as possible and tailored to their needs and preferences. In doing this, we seek to celebrate the meaning of their lives and ensure that their unique legacy is honored.

We will provide care and support for the family and the individual, and serve them with the utmost respect, regard, and grace. We will honor their unique life, their Lifesong.
