Old Colony Hospice, founded in 1979, is one of the first in the Commonwealth of Massachusetts. Old Colony Hospice is a private, non-profit, CHAP accredited hospice and palliative care agency, serving individuals and families in more than 40 communities in Bristol, Norfolk and Plymouth counties. Our goal is to relieve the pain of human suffering in individuals with advanced disease. When you choose Old Colony Hospice, you will feel the difference in our care. Old Colony Hospice ranks in the top 5% in patient and family satisfaction according to the independent national benchmarking organization, Press Ganey. The majority of our staff hold specialty certifications in Hospice and Palliative Care. Old Colony Hospice cares for individuals wherever they call home.....in private homes, nursing homes, hospitals and assisted living residences. We offer free private informational meetings to talk with you and your family to gather information about the services we provide before you need them. Our services are covered by Medicare, Medicaid and most third party payers. Our patients and families never receive a bill. And we work with your primary care physician to coordinate your care. We also offer opportunities for you to help support our mission. Volunteers are always welcome - with opportunities to visit patients, participate in organizing events or supporting some office functions. We also offer opportunities for you to contribute to our mission through financial support for our programs and functions that are not reimbursed by insurance. Without your ongoing support we could not provide the level of community outreach and education, technology support for our electronic medical record and communication devices, accounting systems and our palliative care consultation team, to name a few.
