Palliative Care Nurse Practitioner at Transitions LifeCare

Jackie Francisco has a diverse work experience in the healthcare industry. Jackie is currently working as a Palliative Care Nurse Practitioner at Transitions LifeCare since December 2021. Prior to that, they worked at Optum as a Nurse Practitioner from February 2019 to September 2022. Jackie also served as a Case Manager at Wake Med from March 2014 to July 2018.

Before transitioning into a nurse practitioner role, Jackie worked as an Account Executive at Medical Treatment Systems/KCI from January 2009 to March 2014. In this role, they provided field sales and support for the KCI Wound VAC, working closely with Case Management to facilitate patient transitions.

Earlier in their career, Jackie worked as a Circulating/ Recovery Room Nurse at Raleigh Plastic Surgery Center from December 2007 to December 2008. Jackie was responsible for circulating all OR cases and recovering patients prior to discharge. Jackie also played a role in staff training, pre and post-operative patient education, and served on the Quality Management Committee.

Starting their career in healthcare, Jackie worked as a Critical Care Nurse at Wake Med from May 2003 to December 2007.

Jackie Francisco obtained their Associate of Applied Science (A.A.S) degree in Nursing from Alfred State College - SUNY College of Technology from 1993 to 1996. In 2012 to 2013, they pursued a Bachelor of Science in Nursing (BSN) degree at Winston-Salem State University. From 2016 to 2018, Jackie Francisco pursued their Adult Geriatric Nurse Practitioner degree in Registered Nursing/Registered Nurse from South University.

Celebrating life since 1979, Transitions LifeCare was founded as Hospice of Wake County in 1979 and provides physical, emotional and spiritual care to those living with an advanced illness, their caregivers and those who have lost a loved one. Our services include hospice care, palliative care, home health, caregiver support, and grief counseling. Services are provided regardless of age, race, religion or financial circumstances. Our service area includes eastern Chatham, Durham, Franklin, Harnett, Johnston, and Wake counties, as well as Chapel Hill and Carrboro (Orange County). Guided by community-based boards of directors and supported by a dedicated professional staff, our goal is to provide compassionate hospice care that allows patients to enjoy every moment of their lives to the fullest.
