Mildred Hernandez is a dedicated healthcare professional with extensive experience in nursing and case management. Since June 2022, Mildred has been serving as a Case Management Nurse at Trellis Supportive Care. Previously, from February 2021 to May 2022, Mildred worked as a Charge Nurse and Dialysis Registered Nurse at Atrium Health Wake Forest Baptist, where responsibilities included supervising a team of dialysis technicians and providing comprehensive care for pediatric and adult patients in an outpatient setting. Mildred holds a Bachelor of Applied Science in Registered Nursing from Interamerican University of Puerto Rico and an Associate of Science in Registered Nursing from Inter-American University of Puerto Rico - Metropolitan Campus.

Your Trellis Supportive Care, North Carolina's original hospice and palliative care provider, is comfortable, accessible and comprehensive to address any need that arises. Anyone, including family and friends, can make a referral. And, since there’s no cost to the patient, you or your loved ones can always benefit from any of our services. We also provide help long before hospice is required, which usually translates to less discomfort or pain. Both patients and family receive support and care from a specially trained, dedicated team of clinical staff and generous volunteers. In all we do, dignity, comfort and peace-of-mind matter most. While our care addresses the pertinent medical needs, it also enhances the emotional, spiritual and social well-being. Although most patients receive care in their homes, services are provided to nursing home residents, assisted living communities, group and rest homes, and at the family-oriented Kate B. Reynolds Hospice Home. Counseling and bereavement services are also available to patients and families. Of course, at no cost to them.
