The Service Coordination Team at Tri-Counties Association for the Developmentally Disabled, Inc. supports and advocates for individuals with developmental disabilities. They develop personalized service plans, connect clients with necessary resources, monitor progress, and ensure access to care and community integration in the San Luis Obispo, Santa Barbara, and Ventura counties.
