APG is the leading association in the United States representing physician organizations practicing coordinated care.  TriValley received 5 of 5 stars awarded for achieving all Standards of Excellence.  The five categories reviewed included Care Management Practices, Information Technology, Accountability and Transparency, Patient-Centered Care and Advanced Primary Care.
