Triniti Home Health & Hospice is jointly owned by Guardian Angels Senior Services of Elk River and Cassia, an Augustana/Elim affiliation. From the comforting care we provide in hospice, to the restorative focus we have in home health care, our synergistic teams set us apart from other agencies. Triniti Home Health & Hospice employs a community of health care professionals consisting of nurses, social workers, therapists, chaplains, music therapists, massage therapists, volunteers, and more. Whether you or your loved one is looking for care, our compassionate team is equipped to handle a variety of needs.

We serve people in Anoka, Carver, Dakota, Hennepin, Isanti, Mille Lacs, Ramsey, Scott, Sherburne, Washington, and Wright counties, as well as eastern Stearns, Benton, McLeod and Meeker counties in Minnesota.
