Alison Lohf, MSW, LCSW, has a diverse work experience in various roles and settings. From 2019 to the present, they have worked as a Care Navigation Specialist and Palliative Care Social Worker at Transitions LifeCare. Prior to that, they served as a Geriatric Therapist at Strategic Behavioral Health from 2018 to 2019. In 2016 and 2017, they were an Intensive In-Home Team Lead & Therapist at B & D Integrated Health Services. In 2015, they held positions as a Community Support Team Lead & Therapist at both Yelvertons Enrichment Services and Family Legacy Mental Heath Services Incorporated. From 2014 to 2015, they worked as a Social Worker at Carver Living Center. Earlier in their career, they gained experience as a Clinical Social Work Intern at Care Dimensions and Alliance for Inclusion and Prevention, Inc. In 2013, they worked as a Counselor at Aspire/Massachusetts General Hospital. Alison began their career as a Residential Counselor at Vinfen from 2010 to 2012. Overall, Alison Lohf has consistently worked in roles serving the mental health and social welfare needs of diverse populations.
Alison Lohf, MSW, LCSW completed their education in the field of social work and psychology. Alison obtained their Bachelor's Degree in Psychology from the University of Puget Sound, where they studied from 2000 to 2004. Later, from 2012 to 2014, they pursued a Master's Degree in Clinical/Medical Social Work at Wheelock College.

Celebrating life since 1979, Transitions LifeCare was founded as Hospice of Wake County in 1979 and provides physical, emotional and spiritual care to those living with an advanced illness, their caregivers and those who have lost a loved one. Our services include hospice care, palliative care, home health, caregiver support, and grief counseling. Services are provided regardless of age, race, religion or financial circumstances. Our service area includes eastern Chatham, Durham, Franklin, Harnett, Johnston, and Wake counties, as well as Chapel Hill and Carrboro (Orange County). Guided by community-based boards of directors and supported by a dedicated professional staff, our goal is to provide compassionate hospice care that allows patients to enjoy every moment of their lives to the fullest.
